feat: move app switcher from sidebar to breadcrumbs

Breadcrumbs now show: Pulse ▾ > Your Sites > site ▾ > Page
"Pulse ▾" opens the Ciphera apps dropdown (Drop, Auth).
Removed AppLauncher and CIPHERA label from sidebar top.
